By submitting this form, the requestor acknowledges and agrees to the following:

• Reservation requests must be submitted at least 72 hours in advance and are subject to availability and approval.
• Submission of this form does not guarantee approval.
• The maximum reservation duration is two (2) hours.
• Facilities may be used only for the approved purpose, and attendance must not exceed room capacity.
• Users are responsible for proper use, cleanliness, and any damage to facilities or equipment.
• Food, disruptive behavior, unauthorized equipment, and unapproved commercial activities are prohibited.
• The university and incubator management are not liable for personal loss, injury, or damage.
• The incubator reserves the right to cancel or modify reservations at any time.
• Violation of policies may result in cancellation and loss of future booking privileges.

Once submitted, this request will be reviewed by the Incubator Management Team.
Requestor will be notified via email regarding approval or additional requirements.
